1. Pound chicken breasts thin, using either a plastic food storage bag or between two pieces of plastic wrap.
2. Place slice of ham on each chicken breast.
3. Place slice of cheese on each ham slice.
4. Carefully fold up each chicken breast and insert a toothpick to secure.
5. Place chicken breasts in a 3 1/2 quart crock-pot.
6. Combine milk, soup and pepper and pour over chicken.
7. Cook, covered, for 5-6 hours on low; serve.
